Health and Wellness Coordinator

The Health and Wellness Coordinator works with Pre K-12th students. This position is responsible for planning, designing, implementing and evaluating a comprehensive health and wellness program for the students at HOPE Community Academy. The Health and Wellness Coordinator will respond to students needing medical attention, report concerns, and maintain records.

Responsibilities:

  • Identify, prioritize and provide first aid management and emergency care to students within scope of authority and training in compliance with the MN Nurse Practice Act and current policies/procedures.
  • Administer medications necessary for students to participate in their school day per the MN Nurse Practice Act and current policies/procedures and district policy.
  • Notify parents/ guardians and the appropriate staff member of illness or injury via phone calls.
    • Complete the Incident/ Accident Report Form.
  • Record all visits of students in Infinite Campus. 
    • Non-licensed and LPN staff inform the Supervising Licensed School Nurse and Principal of unusual and/or serious pupil illness or injury, refer to the Supervising Licensed School Nurse about exclusions or re-admissions.
    • Non-licensed and LPN staff notify the Supervising Licensed School Nurse,  Principal, and appropriate staff, such as social worker, of serious incidents including child abuse. 
  • Coordinate and administer screening programs at appropriate grade levels.
  • Assess the health status of students and develop individual health plans as appropriate; provide consultation, education, and support to students, families and staff for student health needs.
  • Serve as a resource to staff for health education purposes; communicate objective health data to student and teaching staff as appropriate.
    • Coordinate and schedule health related training as needed to staff
    • Send out monthly emails about health education or health reminders.
    • Document conversation with the parents/ guardians regarding health concerns.
  • Maintain student health records, monitor and maintain student immunization records based on current state law school entrance requirements ensuring confidentiality.
  • The Health and Wellness Coordinator will maintain current CPR certificates.
  • Provide monitoring of the school environment to prevent and control communicable disease, including but not limited to:
    • Understanding COVID 19 guidelines established by the Minnesota Department of Health and Center for Disease Control.
    • Provide education and assistance with interpreting COVID 19 policies to students, families, and staff.
    • Conduct contact tracing of students and staff. Assist with notifying close contacts.
  • Communicate and consult with the Supervising Licensed School Nurse by phone/visit as necessary.
  • Work with community resources to enhance health services to students when appropriate.
  • Collaborate with the special education team to ensure student success. Duties include attending special education meetings, providing supervision to special education assistants as needed, complete paperwork as required by MN 3rd Party Billing guidelines, and document health assessments for special education.
  • Perform all other related work delegated or required to accomplish the objectives of the total school health program.


Qualifications

  • Licensed School Nurse or RN.
    • Graduate of accredited RN school.
    • Current RN license in the State of Minnesota.

  • Non-license or LPN
    • Experience in a health/first aid position, such as Health Office Aid/ LPN
  • Knowledge of the MN Nurse Practice Act.
  • Current First Aid, CPR/AED certification.
  • Ability to work well with students, staff, and parents/ guardians from diverse backgrounds; interest in working with children/ adolescents.
  • Exceptional time management and organizational skills.
  • Respect for confidentiality of information.
  • Good verbal and written communication skills, proficient in Hmong or Karen.
  • Ability to act quickly and decisively in a crisis.
  • Ability to work independently with minimal supervision.
  • Basic Computer skills.

Preferred

  • Graduate of accredited RN/LPN school
  • License in the State of Minnesota.
  • CNA, EMT, or Paramedic experience.
